110-300-0475(2)a(v) - During interviews with the director and the staff member present during child's sexual abuse disclosure, it was reported that they did not report the disclosure to law enforcement or the department at the first opportunity, but in no case longer than 48 hours from disclosure. 110-300-0475(2)a(vi) - During interviews with the director and the three staff members present during child's inappropriate sexual touching incidents between two or more children on March 23, March 24, and April 8, it was reported that they did not report the incidents to law enforcement or the department at the first opportunity, but in no case longer than 48 hours from disclosure. 110-300-0475(3)b - During interviews with the director and three staff members present during three reported incidents, it was determined that a written incident report was not submitted to the department within 24 hours of a situation involving inappropriate sexual touching on March 23, March 24, or April 8.
